Asthma Review

For patients who are due an annual asthma review.

Please would you answer the questions on the form below and submit it to us.

If your symptoms are deteriorating or you have any concerns, please make an appointment to the respiratory nurse or a doctor as well.

Alcohol Questionnaire

Please answer the questions in the short questionnaire to enable us to assess your alcohol consumption rate and offer advice if necessary.

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Passport signing
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.

Social Visiting for the Elderly

Like all practices, we find time is increasingly limited. Some of you will be familiar that ‘the GP used to call around on my elderly grandparents in the old days’. We regret that we cannot offer the same sort of thing these days, despite the fact that (1) we actually enjoy these visits, and (2) home visits allow us to learn a great deal more about our patients. Home visits are for medical needs only and at the doctor’s discretion.

Dietician and Podiatry

All patients will be referred to the Dietetic department based at Lever Chambers (for patients with diabetes, heart disease, cholesterol problems and obesity, etc.).

The podiatry service is a screening service for Diabetic patients only. Diabetic patients now have screening for their feet at the same time as their retinal screening appointment (eye screen). The podiatrist will then consider the risk involved to your feet and you will either be seen again at the next retinal screening appointment or will be asked to attend an alternate clinic for treatment with the chiropodist. It is important that all diabetic patients attend for an annual check of their feet to look for neuropathy (nerve damage). 

Long-term Monitoring

Monitoring services include:

Diabetes, Blood Pressure, Asthma, Family Planning, Menopause, Cardiac problems and other chronic diseases. 

For patient’s convenience, the doctors and practice nurses all undertake monitoring of patients with these conditions during their normal surgeries. We have found that the introduction of ‘special clinics’ in our working environment did not work very well in the past, for a variety of reasons.

Pharmacist

Our practice pharmacist works alongside the doctors to make sure that you are taking your medications safely. They regularly engage with medication reviews and medicines optimisation. You may get a timely call from him for time to time asking you if you are having any problems with your medication so please do take this opportunity to engage with him and ask questions. He is helping the doctors do their job more effectively.

Cervical Smears and Well Woman Checks

Cervical smears are undertaken by the practice nurses, and you can expect to receive reminders initially from the health authority, and subsequently from our practice nurses if you are overdue or have not attended. Please be informed that it takes the laboratory a variable number of weeks for you to get the result of your smear, and this can be between 6-12 weeks.

The nurses are very experienced, and function as health professionals in their own right. They help the doctors immensely by undertaking most of the routine care of patients with chronic problems, ranging from hypertension to diabetes and asthma. As we are a small practice with excellent cooperation and good relations between all health professionals, the nurses may refer any new problems that are picked up to the doctors.

Practice Nurses

Working alongside the doctors, the nurses provide a wide range of services from cervical smears, injection, advice on contraception and other matters.

Baby Clinic/ Development & Assessment

Vaccinations are done by our practice nurses, and you will receive postal invitations to remind you which injections are due, and when they are due. Child health surveillance services are provided by the surgery.

Baby clinics are now held at Health Centres:

  • Halliwell Health Centre Tuesdays – 13.30 – 15.30
  • SureStart (on Shepherd Cross Street) – Mondays 13.00 – 15.00 for baby weighing and general advice
  • UCAN Centre on Hatfield Road – Drop-in Clinic on Fridays 12.30 – 13.30, for advice only.

Appointments are not necessary for seeing the health visitors at these clinics.
