To register at our practice please speak to the receptionists who can guide you on the process. Prospective patients will be given a 'pack' which consists of a form to complete for NHS purposes (if they do not have a NHS medical card), a questionnaire and a practice leaflet.

You will be offered an appointment to be seen for a 'New Patient Registration Health Check' by one of the practice nurses or healthcare assistant before your record is passed on to one of the GPs. If it is felt that you may be better served by another practice, you will be advised accordingly.

We are only obliged, under NHS regulations, to accept patients that live within the practice's geographical area. If you have stated a preference for a specific doctor, we will try our best to oblige, although this may not always be possible at times of holidays.

Practice Area

The paragraph below outlines the geographical area that we cover, and where we are obliged to visit patients. A significant number of patients live outside these boundaries, and still choose to stay registered with us. We advise all patients outside these boundaries that they should consider registering with a practice nearer their home, and preferably a practice whose area includes your address.

"Starting from the junction of Halliwell Road with Moss Bank Way, the practice area will extend down Halliwell Road till Eskrick Street. The boundary turns right at Eskrick Street, extending along the continuous road till it becomes Avenue Street, until it meets Chorley Old Road. The boundary turns right onto Chorley Old Road at this junction, extendingup towards the junction with Marsh Fold Lane, and follows the road down Wyresdale Road , till it meets Chorley New Road. From this junction, the boundary turns right on Chorley New Road until it meets Devonshire Road, and turns right onto Devonshire Road till it meets Chorley Old Road. At this point , the boundary turns right onto Chorley Old Road till the junction with Captains Clough Road and turns left at this point up till the junction of Captain Cloughs Road with Moss Bank Way. The boundary then turns right at this point, up till the junction with Halliwell Road again, having completed a full circle."
